ABOUT YOU

We’re looking for a Permanent Part Time Case Manager to join our team, working Tuesday to Thursday, 8.30am to 5.00pm.
In this vital role, you’ll deliver person-centred care by supporting clients through every stage of their journey, from consultation and intake to care planning and ongoing wellbeing monitoring. You’ll be a trusted advocate for your clients, building strong relationships and ensuring their needs are met with compassion and professionalism.
You’ll work closely with our service coordination team to make sure your clients receive the comprehensive, tailored support they deserve.
