AS A TARGETED CASE MANAGER YOU WILL… Work with individuals in the community to facilitate Integrated Care. Assess client’s treatment needs. Develop, monitor, and evaluate treatment plans and progress; Facilitate interdisciplinary approaches. Provide intensive case management to individuals with co-occurring disorders. Complete an assessment of the individual’s strengths and needs. Work with clients to complete an individualized strength- based service plan. Guide clients in overcoming barriers to recovery and towards independence and stability! Requirements To qualify for the position, you will need… At least a High School Diploma or GED and two (2) years of experience in the social services field or related field Or a Bachelor’s Degree in the social services A Pennsylvania Certification Board (PCB) certification (CAAP, CAAC, CADC, or CAADC) or actively pursuing this certification during course of employment. A valid PA Driver’s License and the ability to drive, travel, and provide community-oriented services. A willingness to work with integrated team members. Knowledge of or experience with trauma-informed approaches preferred. Ability to work 40 hours per week within agency hours of operation, which are 8am-8pm Mon-Thurs, 8am-4pm Friday with Saturday hours, when applicable. Physical requirements … SEEING: Must be able to see well enough to read reports and use computer. HEARING: Must be able to hear well enough to communicate to clients, vendors, and coworkers. MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community. LIFTING/PULLING/PUSHING: Must be able to lift up to 10 pounds.
