We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services. About the role As a Case Manager, this is your opportunity to role provides long term case management and service coordination with low individual caseloads who have experienced chronic homelessness through the J2SI program. Reporting to the Team Leader, this fixed-term role offers full-time hours and is contracted for six months, located in Geelong VIC. How you will make an impact Manage a caseload in an outreach capacity providing intensive support, service coordination, referral and advocacy to achieve client goals, using persistent and assertive engagement techniques and culturally safe practices with ACCO and First Nation workers. Coordinate case conferences, support and exit planning with referring agencies, apply the Case Management and Trauma Informed Care Frameworks, and ensure risk assessment, safety planning and accurate client records. Monitor and achieve program targets and outcomes using the J2SI measurement and evaluation framework, contribute to continuous quality improvement, and support research and evaluation activities. Foster and maintain positive relationships with TSA staff, J2SI teams, referral and consortium partners, local services and agencies, and actively participate in team meetings, service planning and community practice. Contribute to a safe working environment by complying with OH&S policies, managing information and data in line with TSA frameworks, addressing risks, engaging in professional development, supervision and practice reflection. What you will bring Demonstrated experience in case management, community services, and working with clients experiencing vulnerabilities including homelessness, AOD issues, mental health, disability, and family violence. Ability to work effectively in an outreach capacity, restore relationships, manage challenging behaviours, and respond to crisis situations. Sound knowledge of theoretical frameworks and strategies for supporting clients with complex needs. Strong interpersonal, verbal and written communication skills, with experience in case noting, client information systems, and relevant documentation.
