Case Manager - GYMPIE Classification: SCHADS Crisis Level 2 Location: Gympie Employment Type: Maximum Term – 6 Months Hours: Part-Time, 15 hours per week About the Role We are seeking a compassionate and skilled Case Manager to join our team in Gympie on a part-time, maximum-term basis. In this role, you will provide person-centred case management support to individuals in the community, over the phone, and in person. Your work will support people to achieve housing stability, strengthen social inclusion, and build the skills and confidence needed to sustain tenancies and live their best lives. Service delivery is underpinned by trauma-informed, child-inclusive, strengths-based, and resilience-focused practice, aligned with the mission and values of The Salvation Army. The Case Manager ensures services align with national and state strategic plans and the National Homelessness Stream Model of Care. How You’ll Make an Impact In this role, you will: Build effective, respectful relationships with clients that promote participation, self-determination, and empowerment Provide case management support in line with The Salvation Army’s case management frameworks and standards Conduct comprehensive assessments, identifying needs, risks, and vulnerabilities Work flexibly in a fast-paced environment, balancing brief interventions, case management, and case coordination Undertake ongoing assessment of client needs from intake through to exit Develop individualised case plans collaboratively with clients, identifying goals, interventions, and barriers Implement case plans with active client engagement Support clients to access services, establish community connections, and advocate with service providers when required What You’ll Bring To succeed in this role, you will have: A tertiary qualification in Community Services, Social Work, or a related field (degree level preferred) Relevant experience in a social service environment, including client assessment, brief intervention, and case management A current Working with Children Check A current Driver’s Licence What we offer The Salvos offer eligible employee's a well-balanced package of meaningful benefits including: Salary packaging up to $15,900 tax free + $2,650 meal entertainment benefit.
