Description Casa Serena is a residential substance use, mental health, trauma and eating disorder treatment center for women only. We are currently looking to add creative, compassionate and innovative individuals to the team in the position of Case Manager! As Santa Barbara's only licensed and accredited treatment program for women, we deliver high quality integral care in a collaborative team environment. Benefits Medical, Dental, Vision Insurance 10% 401k Employer Match Paid Time Off Education Assistance Employee Assistance Program Schedule: Sunday - Thursday or Tuesday - Saturday (one weekend day) Primary Purpose and Function The Case Manager is responsible for the full continuum of client care from day of admission to day of discharge. Essential Functions and Responsibilities Provide clinical assessments as directed Manage a fluctuating caseload of clients dependent upon census Facilitate psychoeducation and/or process groups as needed Attend and contribute to weekly Treatment Team Multidisciplinary meeting Coordinate clinical care for all participants in all levels of care Assess clients overall needs, goals and values, to aid in establishing long-term goals Create and facilitate treatment plans according to regulations from governing bodies Liaison with individual clinicians to ensure collaborative care Work closely with treatment team to assist in developing the aftercare and discharge plan Complete all clinical notes in EMR in timely manner Crisis management as needed Other duties as assigned by the Executive Team Requirements Compliance and Confidentiality Adhere to HIPAA, confidentiality laws, and internal privacy policies Maintain compliance with all applicable federal, state, and local laws Ensure adherence to Joint Commission, state licensing, and payer regulations Core Competency (minimum skills, knowledge and experience needed to perform essential functions and responsibilities) Treatment planning Case management Discharge and aftercare planning Group facilitation Therapeutic interventions and crisis management Proficiency in electronic medical record systems (EMR) Ability to communicate clearly, both in writing and verbally Possess a professional demeanor and a collaborative spirit Cultural competency and client-centered care approach Preferred Qualifications RADT, RAC, SUDRC, CADC, CATC, SUDCC or equivalent, or willing to obtain minimum required credentials upon hire Current CPR & First Aid Certification and TB/Health Screen within 60 days of employment (or willing to obtain upon 7 days of hire) Experience in a behavioral health treatment facility Basic computer skills, experience in MS Office or Google Suite preferred Coursework in psychology, addiction studies or social work preferred Valid driver’s license, clean driving record and insurable by our insurance carrier
