Description
The Case Manager works collaboratively with other providers to coordinate services and facilitates the team process to ensure comprehensive, non-duplicative services to the client/family. Creates opportunities for active participation of all providers, care providers, and birth families to develop appropriate goals and assist the child towards meeting goals of their service plans. Effectively facilitates team meetings to ensure appropriate coordination of services, service delivery, monitoring, and evaluation of services. Coordinates/participates as appropriate to develop the treatment/service plan to ensure appropriate service delivery for the client/family. Maintains regular contact with both foster parents and birth parents (when approved by DHHS) to provide support and guidance. Provides direct services to youth and families through providing supervised visitation, support within the home to the foster family/birth family, and one-to-one directed rehabilitation support. Caseloads are small (typically under 10), allowing for more direct hands on work with clients. This position requires a minimum of a Bachelor's degree, experience working with children/families, and a valid State of Maine driver's license and reliable transportation. Must be able to pass background checks including DMV, CPS, APS, and Criminal checks.
Responsibilities
