SKILLS & EXPERIENCE

  • Relevant bachelor’s degree, or a relevant diploma and be working towards a degree. (A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience and a willingness to undertake further training)
  • Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting
  • Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.
  • Understanding of current casework practices for children and young people
  • Current drivers’ licence
    Successful candidates will be required to clear probity checks including National Criminal History Record Check, NSW Working with Children Check & Residential Care Workers Check
Responsibilities

ABOUT THE ROLE

As an Case Manager, you will support children and young people to achieve their case plan goals, including supporting them to be restored to their family, live independently, move to a permanent home or step down to less intensive placement types.
To be successful in this role, you will have experience working with Children & Young People in out of home care and an understanding of trauma and its effects on children and young people. This is a hybrid working role, and regular travel across Western and South West Sydney in suburbs such as Fairfield, Campbelltown and Blacktown is a requirement of this role.

KEY RESPONSIBILITIES

  • Develop and implement high quality therapeutic case plans for Children and Young People in LWB’s care
  • Develop leaving care plans for each young person over 16 to support their transition to independent living
  • Attend shifts within the intensive therapeutic care homes, and work closely with the team
  • Drive the implementation of Leaving care Plans / P4I Plans / Cultural Support Plans and achievement of the case plan goals
  • Actively engage children, young people, their families and link them in with the local community services
  • Participate in an on-call roster as required
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery
