Position : Case Manager
Location : 100 Madison Ave, Morristown, NJ 07960
Shift : 8am - 4pm
Contract : 13 weeks
Pay Rate : $60/hr - $70/hr

REQUIREMENTS

• 2 years of acute care experience
• Case Management, Managed Care. In Patient Case Management is required
• BSN Required
• RN State License required
• BLS Certificate required

• Conduct comprehensive assessments to identify client needs and develop personalized service plans.
• Provide crisis intervention and management to clients facing immediate challenges.
• Facilitate access to community resources, including housing assistance (e.g., Section 8), addiction counseling, and workforce development programs.
• Utilize motivational interviewing techniques to encourage client engagement and goal setting.
• Offer ongoing support through mentoring and guidance, ensuring clients remain on track with their objectives.
• Collaborate with multidisciplinary teams to coordinate care and services for clients.
• Maintain accurate documentation of client interactions, progress, and outcomes in compliance with organizational policies.
• Advocate for clients’ rights and needs within various systems, including healthcare, social services, and educational institutions.
