JOB DESCRIPTION

We are looking for full-time AOD Certified Case Managers to join our team. The Case Manager will be responsible for holding one-on-one case management sessions with clients to address needs related to employment, legal, resources, substance abuse counseling,* and other tasks as assigned. The Case Manager will have a case load as prescribed by the Counseling Supervisor and be responsible for completing individual sessions, group sessions, progress notes, group notes, and assist in aftercare planning. Under the supervision of the Clinical Director, Program Director, and Counseling Supervisor,* the Case Manager will take on tasks as assigned and provide quality care to the clients assigned to them.

MINIMUM QUALIFICATIONS:

  • CADC 1 (Required)
  • Case Management Experience: (Preferred)
  • Knowledge of KIPU (Preferred)
  • First Aid/CPR Certification and Tuberculosis test must be obtained shortly after employment

Responsibilities
  • Manage a case load as prescribed by the Counseling Supervisor
  • Conduct weekly one on one case management sessions with clients
  • Facilitate phone calls between clients and employers, lawyers, parole officers, etc.
  • Assist clients with Family and Medical Leave, Leave of Absence, and disability paperwork
  • Facilitate weekly group sessions
  • Gather resources and develop discharge planning for clients
  • Create clinical documents and templates
  • Attend weekly Treatment Team Meetings
  • Create and update client treatment plans as needed
  • Work with the Program Director, Clinical Director, Counseling Supervisor and therapists to prepare treatment and discharge planning
  • Communicate with client family members in a professional manner
  • Uphold confidentiality and HIPAA guidelines
  • Complete tasks as assigned by Clinical Director, Program Director and Counseling Supervisor
  • Other duties as assigned
