To help fulfill its mission, LOFT is hiring a Full-Time Permanent Case Manager.

JOB SUMMARY

As a Case Manager you will work to coordinate a support network for clients living with mental health challenges, substance use issues, physical health issues, homelessness, and risks of housing loss. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect. Direct support will be provided to clients in service navigation and advocacy, linkages to physical and mental health care, goal setting, crisis intervention, and housing and shelter access.
The Addictions Supportive Housing (ASH) team provides Case Management for adults 16 or older living with HIV or AIDS who face barriers related to substance use and mental health in the Toronto core. The position supports health care and system navigation, ensuring inclusiveness of 2SLGBTQ+ and BIPOC communities, with the goal of community re-integration and personal empowerment.

What You Will Do

  • Develop a plan of support and recovery which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCANs;
  • Provide case management to clients identifying with barriers related to substance use, mental health, physical health, housing and shelter access, immigration, and other intersecting barriers
  • Promote the personal empowerment of each individual through coaching, mentoring, support and follow-up in the development of life skills, including maintaining housing, setting and attending appointments, navigating services, and managing activities of daily living;
  • Establish and co-ordinate a support network of social, family, medical, psychiatric and other community links. Develop a coordinated care plan as appropriate;
  • Provide crisis intervention and assist in the development of safe plans with clients;
  • Mediate issues or conflicts between clients and other individuals that negatively impact clients, striving to teach conflict resolution skills to promote community inclusion and safety;
  • Participate in planning, organizing, promoting and conducting events for the clients in an effort to address their social needs and prevent isolation;
  • Participate in the administration of the program by completing and maintaining the required records, assessments or statistics such as client files in accordance with established procedures;
  • Participate as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities; orienting new staff, students or volunteers; providing other related assistance as required.
