Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. The Case Planner will have to have at least 1 late night per week to meet the needs of working families. Job Responsibilities: Provide weekly change focused interventions with families, primarily in the home, however sessions can take place at the location that best meets the family’s needs during that week, such as at school, the local park, day care center or at Astor offices. Provide counseling to improve parental functioning, address life stressors and improve overall familial functioning. Assess the safety of all children in the home and actively intervene to ensure child safety. Assess all children along the following domains: physical development, cognitive development, emotional development and social/behavioral functioning. Assess the well-being of all family members, including the impact of domestic violence, substance abuse and/or mental health problems on each member’s functioning. Maintain contact with all providers of collateral services, including schools, day care programs, mental health service providers, substance abuse treatment service providers and probation officers as needed. Provide crisis intervention as needed. Develop service plans in collaboration with the family.
