Description
Position Overview: The Case Planner is responsible for all aspects of case management services for a caseload of runaway homeless youth as well as pregnant and parenting youth residing in the Crisis Program. The case planner provides concrete services, resources, counseling and is an integrated member of the RHY Crisis team which strives to ensure improved outcomes for youth. Position Qualifications: BA/BSW degree from an accredited college and experience working with adolescents. Must have at least two years’ experience working with homeless youth or other disadvantaged youth. Familiarity with DYCD programming. Must understand the critical nature of timely and accurate documentation of service delivery and case work practice. Must be a team player.
