As the Case Worker - 1115 Waiver Screener for Catholic Charities of Community Maternity Services, you will play a pivotal role in individuals' lives, making a significant impact through: Duties and Responsibilities – Case Manager/Parent Educator 1. Assess the social, educational and medical needs of Maternity Outreach clients. 2. Act as case manager and coordinate services for Maternity Outreach clients. 3. Provide goal-directed counseling for Maternity Outreach clients. 4. Work closely with Maternity Outreach clients to assure that needed services are provided and utilized. 5. Provide instruction in child development and parent education to parents Duties and Responsibilities – 1115 Enhanced Navigator * Conduct screenings with Medicaid eligible individuals for services under the 1115 Waiver using the approved ACH screening.
