Your Opportunity The Case Worker role provides person-centred, strengths-based support to young people aged 10–24, with a focus on homelessness and early intervention. Working across multiple LGAs, the role delivers assertive outreach and evidence-informed case management, supporting clients to achieve sustainable housing, build capacity, and access appropriate services. The position requires strong relationship-building, sound judgement, and the ability to respond flexibly to the complex needs of vulnerable young people in community settings Your key responsibilities will be to: Work with and respond to referrals of clients across South West Sydney providing ongoing case management and support Create individualized support plans including referrals to community services organisations Promotion of the service in the local community Monitor our clients progress towards achieving their goals Requirements for Success Community Services Degree; or Associate Diploma or certificate with relevant experience; Strong communication, empathy and advocacy skills Have a sound understanding of Mission Australia’s values and code of conduct Well-developed written and oral communication skills, including highly developed administrative skills Current drivers licence. Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
