Description
  Walnut Street Center, Inc is Nonprofit, Human Services agency providing support to Adults with Intellectual Disabilities. We are currently seeking a Case Worker to join our growing team. The Case Worker provides supports for adults with developmental disabilities who are living independently or with their families. Such support services are determined through the individual’s ISP and in ongoing clinical and functional assessments. The goal is to promote individual growth in the areas of choice and decision-making, community membership, household management skills, relationships, and personal accomplishments. This position will ensure that the individuals achieve/maintain independence and control of their lives. The Case Worker balances his/her schedule of visiting individuals in their homes and working in the office taking phone calls from individuals, coordinating services, and participating in team meetings. ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Case Management Skills 2. Provides therapeutic support 3. Provides Safe Environment 4. Provides for community integration and promotes a good public image. 5. Professional responsibility 6. Documentation/Communication. · Documents and notifies management of any issues brought forth that effect the therapeutic care or environment · Participates in meetings and case reviews. 7. Quality improvement. Participates in continuous improvement to increase productivity, reduce inefficiencies and enhance the quality of services provided. 8. Additional responsibilities. Performs other duties, responsibilities and projects as assigned by supervisor or management.
