We are seeking a talented Court-Based Mental Health Navigator, Caseworker to join our team of nearly 500 employees. Mental Health Court Navigators are responsible for supporting justice-involved individuals with mental health needs who are currently in the community. Navigators play a vital role in enhancing services, fostering individual empowerment and planning or stability. Their support may include facilitating connections to mental health and community-based treatment, guiding individuals through the health system, assisting with housing and providing additional referrals as necessary. Not intended to be a long-term program, these voluntary services are not mandated but meant to be available and embedded within the court. Court Navigators will be located at the MCOMH Socio-Legal Center and work in close collaboration with all local courts. The Court-Based Mental Health Navigator Caseworker is community-based and able to meet with clients in the community, at court, jail, or office settings. This role will be tasked with procuring necessary resources, coordinating care planning, completing referrals, etc., based on needs of the individual with whom they are assisting. CCSI is partnering with Monroe County Office of Mental Health (MCOMH) to hire a Court-Based Mental Health Navigator, Caseworker. This position is employed by CCSI and working on behalf of MCOMH. In this role, you will Have a routine presence in Courts throughout Monroe County, providing education and offering support to prospective participants in the moment Receive assigned Navigator program cases, review recommendations and submit referrals based on client need Be a primary team member responsible for coordinating care and connecting to community-based resources In efforts to empower individuals, when indicated, accompany clients to scheduled appointments or other identified community referrals/resources, as personal safety allows. Record progress notes in comprehensive, timely manner and complete all reporting as required by the State Office of Mental Health Recommend adjustments to treatment plans, advocates for equity and system movement to support individual identified needs Be culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services. Communicate in a professional manner as a representative of Monroe County to all providers and partners, including to the local County, City, and Justice Courts. Have working knowledge of the local behavioral health and social services network to assist with matching individuals with appropriate resources to promote health, wellness, and recovery Support community education and/or training requests about program functions Travel as necessary to fulfill responsibilities Have a flexible schedule to meet the needs of the program Performs other duties as assigned Requirements What You Bring Bachelor’s degree in human services field and 2 – 5 years of work experience or equivalent education and work experience (see CCSI's equivalency chart) Additional professional experience with the justice system, criminal procedure law, and/or mental hygiene law are preferred. Required Competencies: To be successful in this position, you will need: Strong communication skills, written and oral, and delivery of correspondence, records, and reports. To demonstrate proficiency in office-related software and technology, able to perform all work-related tasks in a hybrid setting as needed. To build and maintain working collaborative relationships with community stakeholders and providers, not limited to individuals, families, law enforcement, the court system, state agencies, state forensic services, and local service providers. Proficiency of systems of care (i.e. OMH Housing, Substance Use Treatment services, Criminal Justice, Homeless, Health Care). To advocate, provide equitable access and inclusivity to all individuals seeking services from the lens of a person-centered perspective To be culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services. Ability to join staff in problem solving scenarios and provide helpful direction/support Ability to problem-solve and achieve goals both independently and with others Working knowledge of laws and regulations specifically regarding the treatment of individuals with a serious mental illness. Ability to plan, schedule and participate complex cross systems meetings as well as internal team meetings, as scheduled Required for this position are: Satisfactory completion of all required background screenings. NYS Driver’s License and access to reliable transportation for business related travel.
