In this role, you will provide personalised support and mentorship to individuals with disabilities, helping them to achieve their personal goals and enhance their quality of life. You will work closely with clients to develop and implement tailored support plans, promote independence, and encourage community participation.

REQUIREMENTS:

• Relevant qualifications or experience in disability support.
• Strong communication and interpersonal skills.
• Compassionate, patient, and empathetic nature.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation.

• Provide one-on-one support and mentoring to individuals with disabilities.
• Assist clients in developing life skills and achieving personal goals.
• Promote community inclusion and participation.
• Develop and implement personalized support plans.
• Maintain accurate records and documentation.
