UPMC Family Hospice is hiring a casual Home Care Aide for the Lycoming, Union, Northumberland and surrounding areas Hours: 16 hours per period Weekend Rotation every 3rd-4th weekend and 2 holidays per year Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home. Responsibilities: Direct patient care to the patients in the home setting Effective communication to patient, family, and team members Collaboration with interdisciplinary team to ensure a high level of patient safety and customer satisfaction timely documentation within the patient's home Must maintain annual competency evaluation scores as determined by regulatory requirements Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures. Proven ability to facilitate change. Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction. Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide. Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization. Participates in all required agency meetings. Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff. Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards. Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional. High School Diploma or GED - Required One (1) year of experience - Preferred Certified Nurse Aide- Preferred Direct patient personal care in the last 5 years Minimum of 75 hours of Nurse Aide training- Required. If no previous nurse aide training, the applicant may attend the UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
