ABOUT THE ROLE:

We are currently seeking a female Support Worker for a participant in the Croydon area who requires assistance attending appointments and accessing local housing and community services. This is a casual role with flexible hours.
The participant presents with complex psychosocial and physical needs, and a new risk assessment will be developed to ensure the right match and safe service delivery.

KEY RESPONSIBILITIES:

• Provide transport and accompany the participant to appointments and housing services.
• Offer respectful, trauma-informed support tailored to the participant’s needs.
• Maintain clear communication with the Serenity Home Care office and the participant’s care team.
• Keep accurate shift notes and reports.
