Central Coast Local Health District (CCLHD) is looking for a compassionate and skilled Care Navigator to join our dedicated Paediatric Behavioural Triage team. Based in the Women, Children and Families Directorate, this part-time role at Gosford Hospital offers a unique opportunity to guide children, young people, and their families toward the care and support they need most.

VACCINATION REQUIREMENTS

All NSW Health workers are required to comply with the Occupational Assessment, Screening and Vaccination Against Specified Infectious Diseases Policy Directive (PD2024_015). This includes:

  • Category A positions: Mandatory evidence of protection against specified infectious diseases, including annual influenza vaccination and tuberculosis assessment.

Responsibilities

ABOUT THE ROLE

As a Care Navigator, you will play a key role in improving access to services for children and young people (and their parents/carers) with suspected co-morbid neurodevelopmental, behavioural, and mental health issues.

You will:

  • Provide coordinated ‘service navigation’ support, ensuring families are connected to the most appropriate services.
  • Collaborate closely with clinicians, NGOs, and Community Managed Organisations (CMOs) to establish and maintain referral pathways.
  • Support families across various care settings, including hospitals, community services, and through telehealth and telephone-based engagement.
  • Facilitate a smoother experience for families navigating the health and social service systems.
  • Contribute to the development of culturally safe and accessible services for all families.
