Responsibilities

ABOUT THE ROLE

This role focuses on collaborative care, delivering evidence-based interventions and working closely with other professionals, families, and carers to ensure our patients are supported holistically. Working within a multidisciplinary group of healthcare professionals, you will provide high-quality physiotherapy services that support clients in achieving optimal health outcomes.
You will be part of the Transitional Aged Care Program (TACP), which offers time-limited, goal-oriented rehabilitation to individuals transitioning from hospital care. Services are delivered through both community (Transpac) and residential (Transitional Care Unit) settings, targeting improvements in mobility, independence, and overall well-being. Your clinical expertise will help bridge the gap between acute care and long-term support.
