VACCINATION REQUIREMENTS

Responsibilities

ABOUT THE ROLE

  • Deliver high-quality, evidence-based nursing care in clients’ homes, community clinics, and outreach settings, ensuring each person receives safe, effective, and compassionate care that meets their individual needs.
  • Build strong, trusting relationships with clients and their families, providing clear health information, practical guidance, and emotional support to empower them to take an active role in managing their health and wellbeing.
  • Collaborate closely within a multidisciplinary team – including allied health professionals, GPs, and community agencies – to design and implement integrated care plans that address physical, emotional, and social health need.
  • Lead and participate in health promotion and preventative care initiatives across the community, such as education sessions, screenings, and wellness programs, to improve health outcomes and reduce preventable hospital admissions.
  • You will work both autonomously and collaboratively within a multidisciplinary team, supported by clinical leadership and ongoing professional development opportunities.
  • The role operates on a rotating 7-day roster including weekends offering diversity in your daily work and the opportunity to build strong connections within the community you serve.
