VACCINATION REQUIREMENTS

All NSW Health workers are required to comply with the Occupational Assessment, Screening and Vaccination Against Specified Infectious Diseases Policy Directive (PD2024_015). This includes:
Category A positions: Mandatory evidence of protection against specified infectious diseases, including annual influenza vaccination and tuberculosis assessment.

Applicants must provide appropriate evidence of compliance prior to commencement. For detailed information, please refer to the NSW Health Occupational Assessment, Screening and Vaccination Policy.

ABOUT THE ROLE:

As a Community Nurse within our innovative Virtual Care Team, you will provide high-quality, patient-centred care that supports individuals transitioning from hospital to home, and helps prevent avoidable hospital admissions. This role blends clinical expertise with the flexibility of virtual technologies, enabling you to deliver responsive, evidence-based care directly into the community.
You will work collaboratively with patients, families, and a multidisciplinary team to assess needs, develop care plans, and implement strategies that promote recovery, independence, and overall wellbeing.
