GENERAL DESCRIPTION The Consumer Directed Services (CDS) Home Visit Specialist is responsible for facilitating all annual home visits for each Consumer on the CDS program at The Whole Person. The main responsibility of the CDS Case Manager is to conduct the annual visits in order to evaluate the actual living conditions of the Consumer’s home, assessing for any safety concerns and ensuring quality of care as described in the Consumer’s plan of care. The CDS Home Visit Specialist will also go over annual home visit paperwork with the Consumer, including updated contact info, updated releases of information, and a review of policies and procedures. The CDS Home Visit Specialist will also produce efficient, effective, and timely case notes in the CIL Suite Database according to departmental policies and will notify the CDS Supervisor and the CDS Case Manager of any concerns during the home visit. ESSENTIAL JOB FUNCTIONS – LEVEL I 1. Call each Consumer to schedule an appointment for their annual home visit. 2. Visit each Consumer on an annual basis to evaluate their living conditions, ensuring safety and quality of care for each Consumer. 3. Review the annual home visit paperwork with the Consumer, getting updated contact info, updated releases of information, and reviewing policies and procedures with the Consumer. 4. Maintain a running list of all home visits, ensuring that every Consumer has been visited on an annual basis. 5. Inform the CDS Supervisor and CDS Case Manager of any concerns. 6. Hotline any safety concerns to the Adult Abuse, as necessary. 7. Enter case notes and documentation into the CIL suite database in a timely and accurate manner and upload the paperwork into the Consumer’s record documents center. 8. Follow the TWP car policy, ensuring to schedule a TWP car, filling out the required mileage forms, ensure the car has gas, and informing the appropriate supervisors of any issues. 9. Act as backup to the CDS Time And Attendance Specialist, assisting in payroll corrections for Consumers and their Personal Care Attendants, as needed. 10. Regular and predictable attendance. REQUIRED SKILLS · Valid driver’s license. · High school diploma or GED. · 2+ years of proven customer service experience. PREFERRED SKILLS · Bilingual In Spanish. · Knowledge of community services and resources. · Bachelor’s degree in social work or in a related field. · Experience working with individuals who have disabilities.
