Mental Health Kokua (MHK) is hiring a full-time Center Case Manager on Oahu to support adults accessing services at Pūnāwai Rest Stop, a walk-in, guest-facing access point in downtown Honolulu. This role works in a fast-paced, community setting focused on connection, stabilization, and access to resources. As a Center Case Manager, you will engage individuals experiencing homelessness by providing outreach, assessment, and short-term case management support that helps people access services, build stability, and take next steps toward recovery.   What You’ll Do (Responsibilities) * Outreach & Engagement: Build rapport with guests through consistent, respectful outreach in and around Pūnāwai Rest Stop * Assessment & Service Planning: Complete behavioral health assessments and develop person-centered service plans * Resource Navigation: Connect individuals to housing, benefits, medical, and behavioral health services * Care Coordination: Collaborate with nursing staff, case managers, and community partners to support guest needs * Crisis Support: Respond calmly to urgent situations and support de-escalation using recovery-oriented practices * Advocacy: Assist guests with applications, documentation, and follow-up related to services and entitlements * Documentation: Complete timely, accurate electronic records in accordance with program requirements * Team Collaboration: Participate in supervision, team meetings, and coordinated coverage of the site   Center Case Manager Qualifications * Minimum: High school diploma or equivalent (some contracts require a bachelor’s degree) * Preferred: Bachelor’s degree in psychology, social work, human services, or a related field * Skills: Strong communication, engagement, organization, and problem-solving skills * Certifications: CPR, First Aid, and TB clearance (or willingness to obtain) * Fit for the Role: Comfortable working in a walk-in, guest-facing environment with adults experiencing homelessness and behavioral health challenges   Requirements & Work Conditions * Ability to pass a criminal background check and alcohol/drug screening * Ability to lift up to 25 lbs and remain active in community settings * Ability to work assigned schedules, including rotating on-site coverage   Benefits of Joining MHK * Medical & Vision Insurance (UHA or Kaiser) * Dental Insurance (HDS) * Supplemental Insurance (Aflac) * Pet Insurance * 403(b) Retirement Plan * Identity Theft & Legal Protection * Paid Leave & Holiday Pay * Public Service Loan Forgiveness (PSLF) eligibility   Why Mental Health Kokua?
