Our growing Liver Clinic is seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner) to join our multidisciplinary hepatology team. This role provides comprehensive care for patients with a wide range of liver conditions, including cirrhosis, hepatitis, fatty liver disease, autoimmune liver disorders, pre- and post-transplant care, and other complex hepatology needs. The APP will work collaboratively with hepatologists, nursing, and support staff to deliver high-quality, patient-centered care. With the goal after training to see 12-15 patients per day, pending complexity. Responsibilities Monday-Friday 8-4:30 Conduct new and follow-up patient visits, including assessments, diagnosis, treatment planning, and ongoing management of liver-related conditions. Coordinate care for high-acuity and chronically ill patients, ensuring seamless communication across the care team. Order and interpret diagnostic tests, labs, imaging, and specialty evaluations. Manage medication adjustments, including hepatology-specific therapies and monitoring for side effects. Provide patient and family education related to liver disease, treatment plans, lifestyle interventions, and long-term care needs. Participate in care pathways for cirrhosis management, transplant evaluations, and post-transplant follow-up as applicable. Collaborate with inpatient teams to support transitions of care and reduce readmissions. Document all patient interactions in the electronic medical record accurately and in a timely manner. Contribute to quality improvement initiatives, care standardization, and process development within the clinic. Outreach clinic at least once monthly and could involve more pending volumes. Qualifications Licensed Nurse Practitioner (NP) in the state of Iowa Minimum of 2 years of experience in GI/hepatology, internal medicine, or a related specialty preferred. Demonstrated ability to manage complex patients in an outpatient specialty setting. Strong communication, critical-thinking, and patient education skills. Ability to work effectively in a multidisciplinary team. EMR proficiency (Epic preferred).
