Shift: Days Job type: Full Time Summary: The Central Discharge Planner is responsible for the linkage between the multi-disciplinary patient care team and the Skilled Nursing Care facilities, Board and Care facilities and patient transportation resources for the provision of coordination of quality, cost-effective patient care. Responsibilities

The Central Discharge Planner coordinates the transition of patients from the hospital to skilled nursing, board and care facilities, and transportation resources. They act as the primary link between the multi-disciplinary care team and external facilities to ensure quality, cost-effective care.
