Overview We are looking for a Centralized Intake Coordinator to join our team. This position will directly report to the Manager of Centralized Intake and is responsible for overseeing that all referrals are sent via fax, telephone, email and through an electronic platform. This includes oversight and timely response to all referrals issued by subscribing facilities and management and implementation of all aspects associated with acceptance and/or response to eReferrals and coordination with the local Gentiva family brand. The shift is every Sat & Sun 8am-8:30pm.
