Our San Ramon-based team is growing, and we’re looking for a Care Coordinator Manager to play a key role in ensuring that our clients receive seamless, personalized care while supporting our caregivers and clinical staff every step of the way. Position Summary We are looking for dependable, compassionate Certified Home Health Aides (CHHAs) to join our care team. In this role, you will help patients maintain dignity and independence by assisting with personal care and daily living activities—right in the comfort of their homes. You will work closely with our clinical team and under the supervision of a Registered Nurse (RN). Key Responsibilities Provide personal care assistance (bathing, grooming, dressing, toileting, mobility support) Perform light housekeeping and meal preparation related to client care Assist with prescribed home exercise programs and task reminders Observe, report, and document changes in patient condition to supervising RN/LVN Obtain vital signs and assist with medication reminders under supervision Maintain accurate documentation of care provided per agency policy Ensure client privacy while adhering to HIPAA, OSHA, and infection control standards Offer emotional support and companionship to improve overall well-being Qualifications Active California Home Health Aide (CHHA) certification (120-hour CNA/HHA combined training) Current Certified Nursing Assistant (CNA) license in California Current CPR certification (AHA or equivalent) Reliable transportation, valid CA driver’s license, and auto insurance Previous experience in home care, SNF, or hospital setting preferred Kindness, patience, and strong communication/documentation skills Benefits for Part-Time / Per-Diem CHHAs Flexible scheduling to fit your availability Paid training and orientation Shadow visits with experienced caregivers Ongoing engagement through Zingage: caregiver rewards, recognition, and connection Opportunities for internal growth and career advancement Why Join Qualicare?
