Make a Difference This role is part of a team of clinical staff dedicated to implementing innovative care models aimed at enhancing patient care and provider relations. Responsible for promoting efficiency, increasing access, and improving outcomes within the practice through activities such as pre-visit planning, centralized form completion (e.g., FMLA forms), and providing provider in-basket support after clinic hours. Staff members are expected to maintain clinical competencies and support the collection of outcome data related to patient and staff satisfaction, quality of care, and operational efficiency. Additionally, the role involves supporting the well-being of providers and frontline caregivers and creating streamlined patient access. While functioning as an integral team member, the individual will serve as a liaison between the CPN practice and patients. Candidates should be comfortable using various technology platforms. Exceptional Skills and Qualifications This position will be supporting the Pre Visit Planning work for Annual Wellness Visits. Work includes updating the patients chart in the EMR for medical, surgical, and family histories, allergies, immunizations, medications, completes several questionnaires including Health Risk Assessment, Social Determinants of Health, and Depression Screenings. Also reviews current health care gaps, requests/receives/uploads related records. · Diploma from a Medical Assistant program required · Medical Assistant Certification/Registration is required. Qualifying certifications/registrations: CMA (AAMA), NCMA (NCCT), RMA (AMT), CCMA (NHA), and NRCMA (NAHP) * Will be a part of four different teams: MA Afternoon Inbasket, PVP, FMLA, Clinical Practice Improvement. Tasks could involve but are not limited to: Properly navigate through the PVP work queue to complete PVP encounters. Complete a chart review on selected patients, gathering all necessary information prior to contacting the patient. Verifiy patient information by interviewing patient, recording medical history, and confirming purpose of visit. Prepare patient for provider visit remotely, complete all patient assessments and sections of visit encounter related to the type of visit scheduled, and documents in medical record. Communicate with appropriate providers/ office staff all pertinent information related to patient care. Obtain lab, testing, and procedure results from outside sources and appropriately document external results. Communicate with appropriate management or staff to ensure identified problems are resolved. Other duties and responsibilities as assigned (monitor and follow up on voicemail accounts of team members when assigned)
