Summary Certified Nurse Aide Physical Rehab Physical rehab is a 20-bed unit helps patients recover from traumatic injuries, CVA'S, spinal injuries, amputations, and neurological conditions. Certified professional caregiver working under the guidance and supervision of a Registered Nurse. Responsible for providing assigned care according to scope of practice. Has adequate knowledge to approach patients of all age groups. All employees will continually monitor and adhere to safety responsibilities and initiatives. Job Description: Provide compassionate, hands-on patient care, including personal hygiene, bathing, toileting, oral care, and assistance with daily activities. Obtain and record vital signs, weights, intake and output, and report changes or abnormalities to the RN. Assist with patient mobility — turning, repositioning, ambulating, and maintaining proper body alignment. Support patient admissions, transfers, and discharges by preparing rooms, escorting patients, and ensuring comfort and safety. Perform delegated clinical tasks such as specimen collection, blood glucose monitoring, EKGs, and other unit-specific procedures. Assist patients with nutritional needs — setting up trays, feeding when necessary, and maintaining hydration and dietary restrictions. Answer call lights promptly and respond to patient and visitor needs with courtesy and professionalism. Communicate effectively with RNs and team members, participating in hourly rounding and shift reports. Maintain a clean, safe, and organized environment while following hospital safety and infection control procedures. Demonstrate a calm, caring demeanor in stressful or emotional situations and promote patient dignity and advocacy. Participate in department safety training, quality improvement initiatives, and emergency preparedness activities. Requirements: High School Diploma or equivalent Current Kentucky Nurse Aide Certification required or successful completion of first year clinical classes in nursing school and validation of a clinical consistent with level of education. Basic Life Support (BLS) certification is required. If not already certified, applicants must obtain BLS within 30 days of their hire date.
