What You'll Do As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Requirements What You Bring Active CNA License or Certification in the state of employment Possess and maintains current CPR certification One (1) + year Hospice, home health or related organization experience preferred Comfort and sensitivity working with terminally ill patients and their families A calm, dependable presence under emotionally demanding circumstances Strong attention to detail in observation and documentation A valid driver's license and willingness to travel locally for patient visits

Responsibilities

Provide dignified personal care and emotional support to terminally ill patients within their homes. Monitor vital signs, document patient changes, and collaborate with an interdisciplinary care team.
