As a CNA, you'll play a vital role in supporting our patients' quality of life through consistent, one-on-one, relationship-based care in their homes. Compensation:. $17.75-$25.50 based on experience  Benefits: * PTO * Medical, Dental, and Vision * Employee Assistance Program (EAP) * 401(k) Option Shifts: Full time NIGHTS with alternating weekends  Key Responsibilities: * Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting, mobility) * Provide companionship, emotional support, and social engagement * Support meal preparation, nutrition support and light housekeeping * Monitor and report changes in condition * Follow care plans and document accurately using EMR standards Qualifications: * High school diploma or GED * Active CNA certification Colorado * CPR certified * Valid driver's license and reliable transportation * Ability to work independently in a non-facility setting * Home health, long-term care, or hospice experience preferred.
