The CNA performs homemaker services which may include, changing linens on patient bed, preparing meals, feeding patient, and other housekeeping duties for patient as directed in Plan of Care. Assists patient with ambulation and transfer, assists patient with range-of-motion exercises/positioning, reinforces dressings as needed; follows infection control procedures and Standard Precautions against Bloodborne Pathogens and other duties as assigned by RN Case Manager or RN Team Lead according to the CNA task list. Provides emotional support and compassionate care to the patient and family unit. Demonstrates listening skills, offers encouragement when appropriate, and practices patience in voice and manner. Assists other Hospice team members in the instruction of the primary caregiver and family, re: physical care, disease process. Offers support and assistance to the primary caregiver and family during the disease process and hospice program involvement. Requirements of the Certified Nursing Assistant (CNA): High school diploma or general education degree (GED) Illinois State Nursing Assistant Certificate Minimum one year nursing home or home health experience in a supervised setting preferred. Must meet state or federal CNA training and competency requirements. Ability to work independently, make accurate, and at times, quick judgements. Ability to respond appropriately to crisis in home setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes. The expected base salary range for this role is $18.50 - $23.87 per hour. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
