Description Sangre de Cristo Community Care is looking for a Per Request Needed Certified Nursing Assistant to join our dynamic inpatient hospice team in Trinidad, Colorado. Sangre de Cristo Community Care is the Southern Colorado leader in enhancing the quality of life and maintaining the integrity of individuals and their families in need of supportive or end-of-life services by offering symptom management, support, comfort, and compassion. The CNA is a member of the interdisciplinary team and provides patient care as directed by the RN or Case Manager. The CNA participates actively as a member of the IDT to assist in the development and implementation of the patient’s Plan of Care. They provide a full range personal care through regular visits in patient's home and in institutions in accordance with hospice standards and scope of practice as defined with regulatory agencies. In compliance with established Plan of Care and care plans, the CNA provides assistance with personal hygiene, ambulation and exercise and nutritional support for patient respecting personal and professional boundaries.
