Occupational Therapist Assistant Position information Title: Occupational Therapist Assistant Facility: Fidelity Health Care Job Reports to: Rehabilitation Supervisor General Summary/Responsibilities: The Occupational Therapist Assistant directly provides occupational therapy services to patients in their homes or residences. The Occupational Therapist Assistant provides care as ordered by the physician, delegated by the Occupational Therapist, follows policies/procedures and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the implementation of the plan of care as outlined by the physician and supervised by the Occupational Therapist. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members. Qualifications Education Minimum Level of Education Required: Associate degree Additional requirements: § Type of degree: Associate’s or Bachelor’s § Area of study or major: Occupational Therapist Assistant § Preferred educational qualifications: N/A § Position specific testing requirement: N/A Licensure/Certification/Registration § Current Ohio Occupational Therapist Assistant License § CPR Certification Experience Minimum Level of Experience Required: No prior job-related work experience Prior job title or occupational experience: N/A Prior specific functional responsibilities: N/A Preferred experience: One year clinical experience in an acute care setting preferred. Clinical experience should have occurred within the last three years unless currently employed in home care. Other experience requirements: N/A Knowledge/Skills § Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs. § Must be able to creatively adapt multiple environments and situations for successful outcomes. § Must be self-directed and able to work with little supervision. § Computer skills preferred. § Home care experience preferred.

Responsibilities
