Position Purpose: The Outpatient Pharmacy Technician-Care Transitions obtains accurate medication histories from patients admitted to the hospital and assists pharmacist in reconciling home medications with those medications prescribed during their hospitalization. At discharge they assist and manage processes to provide medications to patients prior hospital discharge. Performs other pharmacy technician duties as required in the medication distribution processes to assure safe, efficient medication management. The Outpatient Pharmacy Technician-Care Transitions performs these medication history capture and other tasks while under the supervision and instruction of a registered pharmacist. Key Responsibilities (includes but not limited to): Obtain accurate pre-admission medication histories by interviewing patients, families, retail pharmacies, and other caregivers. Assist pharmacists, nursing, and providers with medication reconciliation throughout the patient’s hospital stay. Coordinate the process of providing discharge medications to patients before they leave the hospital. Support daily care-transition workflow and broader outpatient pharmacy operations including medication procurement, preparation, distribution, and insurance billing. Maintain compliance with AZ Board of Pharmacy regulations and KRMC policies. Serve as a resource for medication-history best practices and help train pharmacy technicians, pharmacists, residents, and students. Participate in quality improvement initiatives to enhance transitions-of-care services. Build strong working relationships with clinical staff across the hospital. Qualifications: Licensure: Certified Arizona Pharmacy Technician Licensure Experience: Prior retail pharmacy experience required Medication reconciliation, third party payer, and patient billing experience preferred Work Environment: Must be able to distinguish color. Occasional exposure to chemicals or biological materials. Role may involve limited exposure to blood/body fluids. Sit 37% of time, stand, 30% and walk 33%. Ability to lift or carry up to 75 pounds.
