CSW or LCSW - Palliative Care 8:30a-5p, Monday through Friday Social Worker Position Summary: As a member of the interdisciplinary team, provides social work services to palliative patients and families as assigned. Patients will reside in counties covered by managed care arrangements in Louisville metro and surrounding areas. Services include: assessment, emotional support, assistance with financial and other social service needs, support and guidance regarding care decisions, legal affairs, counseling and family assistance in planning for caregiving. Social Worker Position Qualifications: Must have Master’s degree in Social Work. Must be a Certified Social Worker in Kentucky or Licensed Social Worker in Indiana, as indicated. One year social work experience in healthcare is preferred. Must have a valid driver’s license in accordance to policy. Use of technology-based tools, including electronic medical record, strongly preferred.
