Description POSITION SUMMARY: The Chaplain provides or arranges counseling to meet the spiritual needs of patients, family members, and caregivers in a way consistent with their beliefs and desires and according to the patient’s care plan. The Chaplain is responsible for assisting in fulfilling the department's vision, which is to touch the lives of every resident and staff member with the transforming power of God’s love as we follow Jesus Christ and serve others. KEY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS: Conducts a spiritual assessment as part of the comprehensive assessment within five (5) days of admission to the hospice program. Assists in developing and implementing the interdisciplinary patient care plan for spiritual care. Provides spiritual support following the ongoing wishes and needs of the patient/family, as well as their religious preferences or definition of spirituality. Works in close collaboration with local clergy when desired by the patient. Maintains and submits written clinical records as the Agency deems, including the initial evaluation, the care plan, and daily notes. · Evaluates the patient's and family's response to and effectiveness of spiritual counseling. · Confirms, weekly, the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits. · Participates in IDG and QAPI meetings and activities. · Conducts religious services of prayer, worship, and rituals for patients/families, as appropriate. · Works with other professionals in resolving spiritual/ethical issues. · Provides spiritual and bereavement support to staff as needed. · Provides consultation and education to the IDG on spiritual care. · Provides funeral or memorial services for patients as requested. · Assumes responsibility for self-development by continually striving to improve his/her Spiritual practice through formal education, workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading. · Performs specific work duties and responsibilities as assigned by their supervisor. MANAGEMENT RESPONSIBILITIES: · Performs those administrative activities necessary for the effective management of the Chaplin Services staff, including provision for the selection and development of employees, employee counseling and motivation, disciplinary action, organization of goals and objectives, and planning, organizing, integrating, and measuring the work performed within the department. · Delegate responsibilities to staff with clear expectations. Requirements CERTIFICATES, LICENSES, REGISTRATIONS · Must possess a valid driver’s license PHYSICAL DEMANDS: The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. WORKING CONDITIONS: Exposed Category: Appropriate Personal Protective Equipment (PPE) is required for every employee engaging in Exposed Category tasks. Frequently exposed to blood, body fluids, fumes, airborne particles, and sharp objects potentially contaminated with blood-borne pathogens or other infectious agents. · The noise level in the work environment is usually moderate. · Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment. · Subject to frequent interruptions by residents, patients, family members, staff, visitors, and/or volunteers. · Subject occasionally to hostile and emotionally upset residents, patients, family members, and/or visitors.
