Description

ADMINISTRATION:

  • Capture all client information electronically
  • Complete assessment reports, request and obtain required supplementary documentation as part of the assessment process timeously
  • Complete intake packs and contracts together with concerned persons as required
  • Keep updated process notes/ records on the process until termination and referral
  • Ensure all relevant documentation and reports for ongoing assessments by other Chev entities or external organizations provided
  • Update Chevnet for new applicants, which could include address book and pre-admissions
  • Maintain statistics
  • Writing relevant reports as appropriate
  • Develop and update resource lists
  • Have access to Chevnet and relevant databases to capture and verify information
  • Record all General and information seeking enquiries, in the event of future accessing of services

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • If you have had experience in Social Work, Life Coaching or Counselling this would be advantageous
  • people’s person with a deep sense of empathy, strong emotional intelligence, and the administrative acumen

Responsibilities

AD HOC TASKS

  • Attending and participation in staff meetings, other relevant committees and meetings (inter- organizational)
  • Liaising with and reporting to the relevant internal and external stakeholders
  • The Consultant team to ensure assessments and referrals are in accordance with Organizational care standards
  • Process applications and admissions to Chev community residential properties and external accommodation placements, in collaboration with Finance Assessment Department and pre- admissions committee
  • Complete financial assessments
  • Complete Home Based Care assessments, and include Home Based Care multi- disciplinary team for collateral assessments as required.
