ABOUT EMBRACE PREVENTION CARE

Embrace Prevention Care delivers virtual, preventive dementia care that helps older adults avoid unnecessary hospital and nursing home stays. Our model has demonstrated a 30% reduction in hospital days with strong patient retention. We were selected by Medicare for their 8-year dementia care GUIDE pilot. For eligible patients and caregivers, care is provided at no cost to patients and unpaid caregivers may receive respite coverage. Medicare pays Embrace a monthly fee per enrolled patient, GUIDE creates a sizable national opportunity across millions of Traditional Medicare beneficiaries with dementia. Embrace reaches them through partnerships with social service organizations, physicians, and home health agencies, supported by rapid patient enrollment specialists. We also plan to extend outcomes-based offerings to Medicare Advantage plans.

Responsibilities
  • Own GTM strategy and execution across B2B2C channels—social service orgs, physician groups (neurology/primary care), and home health agencies—translating GUIDE into partner value and patient impact.
  • Build and lead Partner Development, Growth/Marketing, and Enrollment Operations; establish forecasts, funnels, KPIs, and compensation plans tied to quality and outcomes.
  • Expand market footprint in priority geographies, with emphasis on communities lacking GUIDE providers; operationalize a playbook for rapid partner activation and patient enrollment.
  • Negotiate and manage payer relationships, including a roadmap for Medicare Advantage value-based contracts informed by GUIDE outcomes.
  • Ensure GUIDE fidelity and compliance across workflows (assessment, care plans, medication management, coordination, caregiver support/respite) in partnership with Clinical Operations.
  • Partner cross-functionally (Clinical, Product/Tech, Finance, Compliance) to improve unit economics, retention, and outcomes; inform product requirements and data reporting.
  • Represent Embrace with strategic partners, health plans, and at industry events; contribute to board/investor materials and fundraising narratives as needed.
