The Building Strong Foundations (BSF) for Aboriginal Children, Families and Communities forms an integral part of primary health care. It aims to maintain, improve and promote the optimal health and wellbeing of Aboriginal children aged 0 – 5 years and their families by providing safe, compassionate care through the assessment, planning, implementation and evaluation of nursing care.
The BSF CFHN works in partnership with the BSF Aboriginal Health Worker in collaboration with families and the multidisciplinary health care team to achieve culturally safe, quality care outcomes that meet the goals of the child and family.
The Nurse will utilise the Family Partnership Model as the foundation for the relationship with the family, delivering programs such as the Universal Home Health Visiting, Safe Start and the screening and surveillance program for infants and children as informed by the NSW Health Personal Health Record using evidence based tools and providing opportunistic immunisations according to the NSW Childhood Immunisation Schedule (0-5 years).
The nurse will actively promote, protect and support breastfeeding and facilitate evidence-based groups delivered to meet child health, development and parenting support needs of families within the community.
The BSF Nurse will work in partnership with families to maintain trusting, respectful relationships with parents and challenging behaviours that are not supportive of positive child health and development outcomes, with the need to engage and maintain professional links and referral pathways with the multidisciplinary team and other government and non-government services within the community.
The BSF Nurse will work with internal and external health care services to provide a comprehensive range of support, care and advice. This will be achieved by home visiting and outreach programs.
