JOB DESCRIPTION AND TITLE FOR CHILD HEALTH SUPPORT/PARENT AIDE

Reports to: Program Manager
Provide support services for children and families through provision of supportive counseling, health assessment, health education, behavioral health management, referral to resources, coordination of services and other supports for the purpose of improving health and well-being of children and other family members.
Scope: Perform duties with Maximal independence under the supervision and direction of the program Manager and/ Or Director
Qualifications: 1. At Least 22 years of age.
2. A bachelor’s degree from an accredited college or university with a major study in nursing, mental health, psychology, social work, education, guidance, or related field emphasizing human relations, physical, or behavioral studies.
3. An associate’s degree from and from an accredited college or university with a major study in nursing, mental health, psychology, social work, education, guidance, or related field emphasizing human relations, physical, or behavioral studies and have 2 years of experience working with families or other relevant human services.
4. A High school diploma or general equivalency diploma and have 4 years of experience working with families or other relevant human services experience.

Responsibilities

· Being available for immediate contact so appointments may be scheduled or canceled
· Must be able to carry 25 lbs
· Review each case plan assigned
· Complete Initial Health and Behavioral Health Assessment as well as Treatment Plan within 30 days of case assigned
· Document each family service and all case contacts
· Attend case planning or treatment planning meetings as requested by the CPSW
· Attend Agency in-service training as scheduled and scheduled supervisions with Supervisor
· Transport Child(ren) to supported visitation site when necessary.
· Maintain contact with CSPW as needed regarding case management
· Check work related email at least once daily-providing responses and necessary case updates
Job Types: Full-time, Part-time
Pay: $21.20 - $25.54 per hour
