Job Overview We are seeking a dedicated and compassionate Community Nurse Specialist to join our team at G&P Healthcare. The ideal candidate will play a vital role in delivering high-quality nursing care to clients within the community.
Responsibilities
· Working with our current community nurse specialist, to train our staff to a high standard in all aspects of complex care with children and/or adults in their home environment, Including Airway Management, Ventilation, Tracheostomy, PEG and Seizure management.
· Develop, implement, and evaluate individualised care plans in collaboration with patients and their families.
· Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
· Educate patients and their families about health management, disease prevention, and medication adherence.
· Maintain accurate and up-to-date patient records in accordance with regulatory standards.
· Communicate effectively with multidisciplinary teams to coordinate patient care and services.
· Demonstrate strong leadership by mentoring junior staff and supporting their professional development.
· Manage time efficiently to balance patient visits, documentation, and administrative tasks.
Qualifications
· Registered Nurse RSCN/RN registration.
Experience
· Minimum of 2 Years Post qualification experience.
· Experience with Ventilation and Tracheostomy essential
· Proven experience in community nursing or similar healthcare settings is preferred but not essential.
· Strong leadership skills with the ability to inspire and guide others.
· Excellent time management skills to prioritise tasks effectively.
· Exceptional communication skills, both verbal and written, to engage with patients, families, and healthcare professionals.
· Ability to work independently while also being a collaborative team player.
· A commitment to ongoing professional development and staying current with best practices in nursing care.
