You will work as part of the Children’s Community Nursing Team and Children’s Nurse Specialist Team, supporting and managing Children within the community and in the hospital with diagnosed Respiratory condition.
To assess, plan, implement and evaluate care delivery that supports the child/young person to remain in the community setting and where possible prevent hospital admission. Where hospital admission is required, you will be able to support the inpatient ward team to meet the needs of the child or young person and support with timely discharge.
It is expected that you will work collaboratively across local and national networks for asthma, cystic fibrosis, long term oxygen and tracheostomy, ventilation at home management.
You will participate in service development and collaborative working across health, social care and multi-agencies for patients diagnosed with complex respiratory conditions.
