POSITION SUMMARY:

The primary responsibility of this role is to support seniors and older adults with chronic illnesses in managing their financial benefits and coordinating in-home support services as well as home and community-based care under the Choice for Independence (CFI) Program, in accordance with He-E 805 guidelines for the Choices for Independence Waiver.
This position works Hybrid with mandatory attendance at Gateways’ Nashua office a minimum of two days with 50% of time spend in office or in the field, for monthly staff meetings and other required trainings. Staff are required to be accessible via phone and Microsoft Teams during work hours and maintain an up-to-date calendar within Outlook. During remote work, individual must be able to meet in person if needed and can’t have any other care responsibilities during remote work.
This position is for a caseload of 40 individuals serving clients living in Hillsborough County.

QUALIFICATIONS, EDUCATION AND EXPERIENCE:

  • Proficient in Microsoft Windows applications including a working knowledge of Word, Excel, Outlook, along with relational data base knowledge. (Microsoft Access and/or Harmony preferred)
  • Possess reliable cell phone and internet access to provide case management to individuals from a remote (non-Gateways) location.
  • Bachelor’s degree preferred in related field: or equivalent combination of education and/or experience.
  • Three years of prior geriatric casework or related geriatric-based care experience: experience with end-of-life services is preferred.
  • Strong belief in provision of community services for the aging population and people with disabilities.
  • Communicate constructively to management and staff on problems, ideas, or any other insights to assist in defining new best operational practices.
  • Possess reliable transportation, valid driver’s license, and applicable vehicle insurance for business travel.
  • Strong writing skills.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

  • Occasional standing
  • Frequent sitting, minimum of 5 hours per day
  • Occasional walking, bending, and twisting of spine
  • Light lifting and carrying, less than 30 lbs.
  • Visual acuity-worker is required to have close visual acuity to perform job functions
  • Climbing Stairs routinely for home visits purposes
  • Pushing and or pulling under 75 lbs. without assistance infrequently
  • Driving routinely and the ability to transport oneself
