Job Details Level: Entry Job Location: Comanche Nation CHR Program - Lawton, OK Position Type: Full Time Education Level: High School Diploma/GED Salary Range: $15.00 - $18.00 Hourly Travel Percentage: Daily OPENING DATE: November 15, 2025 CLOSING DATE: Open Until Filled POSITION SUMMARY: Community Health Representative Aide (CHR) is to serve as an advocate and facilitator for the individuals/families of Native American descent. Our program offers support through outreach, education, informal counseling, social support, patient-centered care and advocacy services that improve the health and wellness of the Native Community. The CHR Aide provides direct patient care in patients home and safe patient transport services under the supervision of licensed nursing staff and in accordance with facility policies and procedures. This dual-role position supports patient mobility, comfort, and safety by assisting with daily living activities, performing basic nursing tasks, and ensuring efficient and compassionate transport of patients to and from diagnostic, treatment, and surgical areas. The CHR Aide plays a vital role in maintaining a clean, safe, and supportive environment, demonstrating professionalism, teamwork, and excellent communication skills while promoting patient dignity and well-being. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, light house work, and toileting. Measure and record vital signs, intake/output, and other routine observations. Safely lift, transfer, and position patients using proper body mechanics and assistive devices. Transport patients between departments via wheelchair, stretcher, or bed, ensuring safety and comfort at all times. Communicate effectively with patients, patients’ family, nursing staff, transport dispatchers, and other departments regarding patient needs and schedules. Remain flexible as schedule is subject to change quickly. Pick-up and deliver medications to patient’s home. Advocate for patients needs and report any concerns immediately to supervisor/s. Sit in at patient appointments when requested. Clear documentation of all patient care and encounters. Maintain cleanliness and readiness of transport equipment with no heavy perfumes. (vehicle, wheelchairs, stretchers, lifts). Adhere to infection control, safety, and confidentiality standards. Attend continued education/training as required by the job to best understand and take care of patients. Support an environment of compassion, respect, and teamwork. Keep licenses and certifications up to date to remain complaint. Other duties as assigned. REPORTS TO: Will report to CHR Director/Licensed Nurse and Lead CHR generalist. PHYSICAL DEMANDS: Use of equipment: Operating mechanical lifts, slide boards, or gait belts safely. Must be able to lift up to 50 pounds and assist clients with mobility as needed. Proper body mechanics using correct lifting techniques to prevent back, shoulder, or joint injuries. Defensive driving and the ability to drive long distances Ability to get in and out of vehicle multiple times a day. WORK ENVIRONMENT: CHR staff is often on the road with patients in a work provided vehicle. Travel is required locally and to surrounding areas up to 2-hour travel distance away. Travel is required in all type of weather. When not on the road staff is in an office setting. Work days are Monday-Friday with varying 8-hour shift with a mandatory 1-hour lunch. QUALIFICATIONS: • High school diploma/GED • Registered and in good standing on the Oklahoma Nurse Aide Registry. • Valid Driver License. • Experience in home health, long-term care, or community health preferred. • CPR and First Aid certification required. • Must successfully pass a background check and drug screening.
