Location: Jacksonville, FL 32256 (Primarily office-based with patient outreach via phone and coordination activities) Your Impact in This Role As a Chronic Care Coordinator, you’ll play a key role in supporting geriatric patients by ensuring continuity of care, managing chronic conditions, and helping patients navigate the healthcare system.
