POSITION SUMMARY:

The Chronic Care Enrollment Specialist plays a critical role in identifying and enrolling eligible patients into chronic care management (CCM) programs. This position supports care coordination efforts by conducting outreach to patients with chronic conditions, explaining program benefits, and obtaining necessary consents. The Enrollment Specialist works closely with clinical and administrative teams to ensure appropriate and timely enrollment, promoting patient engagement and improved health outcomes.

EDUCATION AND EXPERIENCE:

  • Minimum of 1-2 years of experience in a healthcare setting, preferably in care coordination, patient outreach, or medical office administration.
  • Experience with Medicare or chronic care management programs is a strong plus.

SKILLS AND COMPETENCIES:

  • Strong communication and interpersonal skills
  • Excellent organizational and time management abilities.
  • Proficient in EHR systems and Microsoft Office Suite (Word, Excel, Outlook).
  • Knowledge of HIPAA regulations and patient confidentiality standards.
  • Ability to work independently and in a team-oriented environment.

  • Review electronic health records (EHRs), reports, and referrals to identify patients eligible for chronic care management programs.
  • Conduct outbound calls and follow-up with patients to explain CCM program offerings, eligibility criteria, and benefits.

  • Accurately enter and update patient information and enrollment status in internal systems or care platforms.
  • Collaborate with care coordinators, nurses, and providers to ensure seamless transition into the CCM program.
  • Maintain accurate and timely documentation of all patient interactions.
  • Educate patients on available resources, program expectations, and care coordination processes.
  • Assist in tracking enrollment metrics and reporting outcomes
  • Address patient questions, concerns, or barriers to enrollment, escalating issues as needed.
  • Stay informed about changes in chronic care program requirements, payer guidelines, and regulatory updates.
