As a Chronic Care Manager (CCM), you will be instrumental in coordinating comprehensive care plans for patients with complex health needs. Reporting to the healthcare team, your role emphasizes patient interaction and effective communication to ensure optimal patient service and care. QUALIFICATIONS

  • Candidates must have strong communication skills, experience with EMR, multiphone lines and at least 1 year of medical office experience
  • Must be able to call and communicate well with patients over phone
  • Must also have basic knowledge of chronic medical conditions such as hypertension and diabetes

Responsibilities
  • Working closely with our Care Manager, the CCM assistant will provide outreach calls to patients enrolled in our Chronic Care Management Program
  • The MA will be responsible for assessing chronic conditions via phone, medication reconciliation, reviewing and updating care plans under the direction of the physician and coordinating with the care team to help patients meet their goals
  • In addition, the MA will meet regularly with the care team and providers to review care plans for patients and discuss any potential clinical concerns identified
  • The MA will be expected to reach out to all assigned patients daily and to document all calls in the EHR to ensure compliance with the CCM program
  • You will need to be able to call in prescriptions and make appointments
  • This is a full-time position working Monday through Friday
